Accident or Sickness

• AIG Accident and Sickness Claim Form/Global – Proof of Loss (both sides completed including physician’s signature)
• Supporting hospital and medical bills (or have physician fill out and sign “part B” on reverse side of form, itemizing charges)
• Police report, if applicable
  
  

• AIG Dismemberment Claim Report (both sides completed)
• Police report, if applicable
• Medical reports
• Any other pertinent items specific to the case
  
  

• AIG Proof of Death form completed and signed by employer and by beneficiary as indicated
• Death Certificate–certified copy or original
• Police report
• Autopsy report, if applicable
